Keuffel Esser Log Duplex Decitrig Slide Rule Box Leather Case Manual 4081-3SB.
Please see pictures. Slide rule is in good condition with one cracked magnifier. Leather case with chamois lining is in good condition. Slide rule manual (1955) and slide rule guide and catalog by Don Herold (1958) are bent with some foxing and tears. I don't know if the latter is original, but it will be included as found. Box is very damaged. The belt carrier is missing.
The item number is 4081-3SB; I haven't been able to figure out what the "B" means. Otherwise, it's a 4081-3S per the catalog, just missing the belt carrier.
